Coordinated Services of Idaho provides Targeted Service Coordination and Residential Habilitation (ResHab) Services in Post Falls, Hayden and Coeur d’ Alene area. We love working with adults on becoming as independent as possible. Adults who qualify for Medicaid services with the DD Waiver can contact us directly.
Our dedicated targeted service coordinator is here to support families and individuals in accessing vital quality-of-life resources and ensuring seamless coordination of necessary care and services. We are committed to assisting adults with developmental disabilities, offering a comprehensive range of services. These include Adult DD Waiver assistance, as well as providing valuable resources for housing, transportation, vocational support, social activities, medical equipment, and a plethora of other essential needs. Your well-being and empowerment are at the heart of our mission, and we are here to guide you every step of the way towards a better, more fulfilling life.
The Residential Habilitation or Supported Living program at Coordinated Services of Idaho is designed to provide tailored assistance to individuals with disabilities who reside in their own homes within our community. Our services are customized to meet the specific needs of each individual and include supervised care. Participants are given the freedom to choose their living environment and services that suit their unique requirements, enabling them to live independently and thrive within our community.
Our compassionate and professional staff offer ongoing assistance in with activities of daily living including menu planning, grocery shopping, cooking, household tasks, etc. Staff also support clients in community involvement. The ResHab program provides flexibility, allowing participants to live on their own with staff support as needed.
Our staff also assists participants in reaching long term goals, such as developing health and safety practices, maybe get your driver’s license, take art classes. We are committed to promoting independence and helping individuals with disabilities achieve their goals.
